CONSENT & LIABILITY WAIVER

Hold Harmless: By participating in J. Y Sports Programs, I agree to release and hold harmless J. Y. Sports its officers, directors, employees and agents, Cobb County Parks and Noonday Park from any and all liability, losses, claims, actions, costs, including attorney fees resulting from injury to person or damage to property arising out of my participation in this event.

Photograph Authorization; I hereby give J. Y. Sports and it’s affiliates, permission to photograph me during the season and it’s related activities, and to use those photographs for any purpose, including but not limited to commercial purposes, J. Y. Sport’s advertisements and promotional materials.

Eligibility Certification; I certify that in accordance with the rules of MLS and FIFA governing amateur status, I am eligible to participate in the practice sessions and games. (No ineligible players will be allowed to particpate).
